The Healthy Sacramento Coalition, led by the Foundation, connects local work with national conversations through funding from a five-year Community Transformation Grant from the Centers for Disease Control and Prevention and is comprised of more than 100 community partners representing neighborhoods in Sacramento County that experience disproportionately poor health outcomes.
